Important Sign-Up Information

The link below will connect you to the software program used to register you and your child(ren) with Unique by Design Pediatrics. The first two pages of the sign up require YOUR information as the parent. You are called the MEMBER. At the bottom of the 2nd page – there is a box that is pre-populated that says “DO NOT ENROLL ME” – this establishes you as the parent/the responsible party and not the patient. The next page will allow you to enter your child(ren)’s information. You will be required to enter an email & phone number for your kids (please use YOUR email & phone number unless you have a teen that you want to have independent contact with me). In order for me to send you access to their portal (electronic medical record), an email is required and for me to be able to electronically prescribe medications the system requires an email & phone number.
